Form (excel-24kb) - Please fill out the form, using an address you
would like your reimbursement check sent to (if you would like the Treasurer
to bring the check to the next meeting, send him an e-mail or give him
a call). Include any receipts or copy of receipts with the form.
You can either bring the form to a meeting, fax it to the Treasurer's office
or e-mail it to him (see above). Contact the Treasurer if you have
any questions.
